What to expect from your first home visit

The first visit is a conversation, not an inspection. Here is how it runs, what to have ready, and the questions worth asking us.

A care manager going through a care plan with a family at a dining table

The first home visit is where care starts to feel real. It is also the part families worry about most. In practice it is a conversation at your kitchen table with our Care Manager, usually lasting about an hour.

What happens

  • We listen first: what is working, what is hard, what matters most to you
  • We walk through the home with you to spot any safety concerns
  • We talk about your health, medications and any instructions from your doctor
  • We explain what we can offer now and what will depend on your assessment
  • We agree on next steps and a date for your care plan

What to have ready

  • A list of current medications
  • Any letters from My Aged Care or your doctor
  • Names and numbers of family members you want involved
  • A list of questions, however small

Questions worth asking us

Who will my carers be, and will they be the same people? Who oversees clinical care? How do I raise a concern? What happens if my needs change? We would rather you ask now than wonder later.

Next step

Let's talk about the care you need

A short, no-obligation conversation with our Care Manager is the best place to start. We will listen first, then tell you honestly what we can do.
