Staying connected to your community
Loneliness is one of the biggest risks to health in later life, and one of the easiest to do something about. Our social support is about company and connection: a regular visitor to share a cuppa with, a lift to church or the club, a walk in the park, or help using the phone or tablet to see the grandchildren.

- Family carers who want their loved one to have more company during the week

How it works
- 01
We find out what you enjoy, what you miss and who you would like to see more of.
- 02
We match you with a support worker who shares your interests or language where we can.
- 03
Visits and outings are planned around your energy and your calendar.
- 04
Your Care Manager checks that the support is actually making a difference, and adjusts it if not.
Who delivers it
Social support is delivered by community support workers with a current police check and first aid, including workers who speak Filipino languages and other community languages.

Let's talk about the care you need
A short, no-obligation conversation with our Care Manager is the best place to start. We will listen first, then tell you honestly what we can do.
