Care management that keeps everything on track

Good care is coordinated care. Your Care Manager is a single point of contact who understands your goals, arranges every service, speaks with your GP and allied health providers, and reviews your plan as life changes. For families, it means one phone number and one person who knows the whole picture.

How it works

  1. 01

Your Care Manager visits you at home to listen, assess and understand what matters most to you.

  2. 02

Together you agree on a care plan with clear goals, services and a review date.

  3. 03

Your Care Manager sets up the roster, briefs the care team and confirms the first visit.

  4. 04

You hear from your Care Manager regularly, and you can call them whenever something comes up.

Who delivers it

Care management is delivered by our Care Manager with support from the clinical and quality team. It is a core part of how we deliver every other service.

Questions

Common questions about care management

Both. Every client has a Care Manager. For people with more complex needs it is also a service in its own right, with dedicated time for coordination and reviews.
